Set in Italy during 2001, two years after the events of Diamond is Unbreakable, the series follows the adventures of Giorno Giovanna, the son of Dio Brando (from Phantom Blood and Stardust Crusaders) but conceived with Jonathan Joestar's body, who joins the criminal organization Passione in the hopes of becoming a gangster (or "Gang-Star") and taking control of the organization in the name of reform.

This is the case for Giorno's and Fugo's backstories. Giorno is neglected and physically abused by his stepfather, and Fugo is (almost) sexually assaulted by a teacher.
Drugs are sold by the Passione Mafia. Bruno Bucciarati and Giorno Giovanna have an extreme hatred for drugs which prompts them to overthrow the boss, kicking off the plot of the story.
Episode 9, 14:30 to 14:55 -- Formaggio uses his stand to shrink his cat and trap it in a glass bottle containing liquid. The cat is visibly distressed.
Episode 11, 11:50 to 15:50 -- Narancia is shrunk and put in a bottle with a spider. The spider is relatively huge, and manages to bite, web, and almost kill him, but fails.
Episode 12, 11:30 to 12:20 -- A professor corners a young boy and starts to undress, though the boy attacks him and escapes before anything can happen.
Episode 17, 6:30 to 6:45 -- A woman wakes up after talking to an unfamiliar man, and, although she hasn't been, still checks to see if she was raped.
Episode 22 - A character cuts his tongue off and starts chocking (on the blood). This leads to another character having to perform a cricothyrotomy using a pen.
Various characters have their arms amputated on numerous occasions throughout the series. The main cast has a healer who is able to repair these injuries. There are also a couple of incidents where some characters lose a part of or their entire foot.
Episode 6-7: Although not a hanging as in standards of a noose, Mario Zucchero has his head detached from his body via Bruno's stand: Sticky Fingers, and has his eyelid attatched to a fishing hook and hang him from the pole of a boat, all while still being alive due to said stand ability.
Bruno is briefly seen having seizures after the Boss heavily injures him, and another time toward the end of the series when his body is shot a few times.
Borderline. A main character used to be a cop and this is shown in flashbacks. He is framed rather sympathetically, though the corruption of the police department is also emphasized.
In a flashback, a character is briefly seen at his mother’s bedside in a hospital. Later, a character is shown on an autopsy table, and is conscious as his body is autopsied.
Episode 34: After the entirety of rome swap bodies, Trish makes comments about Mista's body smelling, while Mista starts going insane over not having his you know what anymore. Giorno goes back to his body after Narancia dies in there and leaves his soul, and the rest of the living cast besides Bruno, who ascends after leaving swap back to their bodies.
There is a scene where a main character is shrunk down to a very small size and trapped inside a bottle with a spider, making the spider appear much bigger than it actually is.
Polpo accidentally shoots himself after being tricked by Giorno.
Another example includes a Passione member committing suicide by shooting himself after passing information to Bucciarati’s gang. This is only shown in a replay via the Stand, Moody Blues.
In the Grateful Dead arc, a baby or toddler cries for its mother, who has presumably aged and died from the effects of The Grateful Dead. Quite an upsetting scene.
Polpo is an extremely obese prisoner, and there are derogatory marks made by characters where they consider how he could have fit through the cell door, and how they would likely have to cut him into pieces to fit his body in an incinerator.
Episode 19, "White Ice", very beginning of the episode -- a car is intentionally driven off a bridge to land in water. The car does not sink.
Episode 30, "Green Day and Oasis Part 1" -- several car crashes occur in the background.
The fight against the stand Notorious B.I.G takes place on a plane, which eventually mostly crashes into the ocean. All of the main characters survive this.
Vento Aureo is significantly more violent and graphic than other parts in the series. Characters are stabbed, shot, murdered, have limbs torn off, etc. [spoilers] Specific examples of this include two characters getting punched through their bodies, two characters being impaled, and a graphic fight wherein one character is made to vomit up razor blades.
