In order to get an achievement, both a squirrel and a bird must be burned alive. Pyhrokenisis can also be used on enemies until they die, but it isn't required. The memory vault "Milla's Children" also shows that the orphans Milla used to work with all died when the building caught fire while Milla was away getting groceries. This memory is pretty well hidden and isn't required to finish the game.

"Insane" and "Crazy" are used pretty often. The main character, Razputin, is called "Raz the Sp*z" by a bully in a cutscene after the first level. The four levels that take place at a mental hospital have implications that Raz has cured the patients of their mental illnesses, but none of the patients are made the butt of jokes and are meant to be sympathetic. (Boyd has paranoid delusions, Gloria has mood swings, Fred has an alter/split personality/maybe possession of his ancestor Napoleon, and Edgar seems to have depression or PTSD over a high school breakup.)

In the memory vault "Sasha's Second Sight" in Sasha's Shooting Gallery, it is shown that when Sasha was reading his grieving father's mind, he saw a memory of the two having sex. The image is not explicit, only showing Sasha's mother's naked shoulders and a little bit of cleavage, but Sasha is so mortified by this that he leaves home.
