A lot. I wouldn't watch if heroin triggers you. Basically every scene with Isaac the painter is about drugs or tangentially involves drugs.
He's addicted to heroin and suffered a relapse before the events of episode 1, and he resists his GFs help trying to get him into rehab because the drugs enable his powers
In episode 2 his girlfriend tells him he overdosed and threatens to break up
At the end of ep 2 it's implied he's on the edge of relapse and we see drug paraphenelia
E3 he uses onscreen
He detoxes in E8 but then HRG forces him to use again (onscreen, very triggering)
E17: Isaac relapses after losing Simone (it happens in the scene right after Mohinder's prisoner escapes)
