Not on-screen, but in the latter part of the game a character mentions someone abusing horses offscreen.

You do have to shoot switches shaped like eyes with arrows, but it is not graphic.

Link takes damage from fire, so he can die from it. There is no added graphicness, however, as the same death animation plays, just with a non realistic fire effect.

The shadow temple takes place in a literal dungeon where people were tortured, with medieval torture devices as set pieces, though no torture takes place during the game, on or off screen.

Princess Zelda is kidnapped, but it is not a violent exchange; she is simply trapped inside of a crystal.

The ending looks mostly optimistic but becomes sad in retrospect if you play the sequel.

A little bit here and there.
In most versions of the original game, there are couple of areas where you can find blood-stained floors in what appear to be abandoned torture chambers. (The blood was completely removed in the 3D remake.)
And in the cutscene following the first half of the final battle, the antagonist spits up (what is supposed to be) 'blood'. However the color of the 'blood' is dependent on which version of the game you're playing. In earlier copies of the original game, it's red. In later copies (including all re-releases and the 3D remake), it's green. I believe there are also a few copies floating around where the color depends on which tunic Link is wearing at that moment (red, blue, or green).
