There are some scenes that may be too much to those are sensitive. Fingers are severed, throats are graphically punctured and slit, a torture victim's stomach is painstakingly sliced open to remove an object inside, people are impaled, etc

Harley says that John is a two-timer for having gone with Bruce's plans instead of her own if he gets to thwart her on the bridge. While not technically cheating, you are able to make John jealous of you in earlier episodes by getting close to Harley after he tells you he's in love with her.

A mentally-ill person (with symptoms largely presenting as some form of BPD, but ranging from hallucinations, manic states, delusions, psychosis) is shown to have extreme feelings that can erupt into violence.

There are MANY scenes of heavy violence with blood and gore, a scene with bloody body parts, a part where Bruce gets impaled, a one scene where the player must tend to an open wound. Very gross.
