One of Batman's worst hallucinations is when he sees Barbara Gordon commit suicide, under the influence of Scarecrow's fear toxin. Thankfully it didn't actually happen, but the imagery at the time is as intense as it gets.

Batman is outed as Bruce Wayne to the world. Shortly thereafter (if you have completed all the Most Wanted missions) Batman initiates the Knightfall Protocol, self-destructing his mansion with (supposedly) him and Alfred inside. 100% completion also reveals an ending in which a new vigilante takes over Batman's position, who may also be using Scarecrow's fear toxin...

In one of the more unbelievable breaks from reality, while you can absolutely hit criminals (as they are all that's left in Gotham) with your Batmobile, they are apparently Tased and knocked away and out, and not killed. Still pretty messed-up.
