In S1E2, about 38 minutes and 50 seconds in, Dr. Hanover mentions in his monologue that Edmund r*ped one priest.

In S1E2 about 43 minutes and 36 seconds in, you see a flash back of Edmund attacking the Monsignor from the point of view of Father Andrews hiding under the bed. You can hear panting, but you can't really see Edmund. It's over in under ten seconds.

At the timestamp 36:30 (Moments after all the nurses are lined up at the nurses station, and Nurse bucket is talking about how the place should be spotless enough for her to eat runny eggs off of any surface) in the 1st episode of the 1st season a patient who is seen screaming and fighting against the nurses trying to hold him down, is forcibly injected with something that appears to knock him out, or at the very least sedate him. It's not a super long scene, but could still be upsetting, nonetheless. The scene only lasts about 30 seconds so it's safe to look at the 37-minuet mark, or when Nurse bucket begins talking about how the nurses need to perform their duties in a very professional manner.

Not quite eye mutilation, but in episode 2 there is an ice pick lobotomy scene, and the ice pick goes under the eyelid to hit the bone there, not damaging the eye itself. The most graphic, close up part of the procedure is shown from about time stamps 19:15 to 19:40. The procedure then picks back up starting at 20:30 from further away with much less detail shown, but you can still hear noises and a description of the procedure, ending at around 21:20.

*SPOILER* *CONTENT WARNING: TORTURE* Early in S1E3 a lesbian woman is given experimental “treatment” for her homosexuality without her consent or foreknowledge. The character is placed in a warm bath and then trapped via a metal contraption. The water temperature is raised until almost boiling. This goes one for a couple agonizing minutes then she is transferred to an ice bath. She is clearly in pain and begs until she is no longer able to do so. I thought she was going to die.

one of the main characters has DID (which they refer to as multiple personality disorder) and the host (the one they consider the "original") is portrayed as sweet and timid while the alters are portrayed as annoying, rude and in some cases dangerous. the treatment revolves around getting rid of th0e alters rather than managing them and learning to live as a system. i don't speak for everyone with DID but most that i know see each alter as a whole person, and treatment usually revolves around each alter learning to function together

It depends, in S1E2 a child that is presented as being very shy, introverted, nerdy is given a lobotomy for daydreaming and innatentiveness. The idea that he was autistic came to mind (I am also autistic and a psychologist), but it is nothing explicit and it is not shown.

Ratcheted manipulates a patient into ending his life with a letter opener in S1 E1 the maniuplation begins at 44.19, the suggestion of suicide begins at 45.27, it is alluded to that he begins to take his life at 47.05 but this is off screen, his body is found at 48.08.
