The film follows a mafia hitman from youth to old age. We also see his mafia colleagues grow old and frail, and a couple of them do die of cancer. I would disagree with the other comment that it's not a major plot point - aging and death are major themes, and the last half-hour of the film especially lingers on this. It could certainly be upsetting if you have any anxieties or triggers related to cancer.
