one of the primary enemies in the game are large lizard predators. they don't have wings and aren't dragons, but they are like giant alligators or komodo dragons and you do fight against them quite often, which may result in their death. other creatures can also kill the lizards and lizards can sometimes fight each other.

An area called Shaded Citadel is heavily populated by little spiders who will crawl around in the dark that can attack you, but this area is technically optional. In harder difficulties however there will be more common spider enemies in many of the areas.

In the DLC of the game, during one of the character's campaigns (gourmand), you can find a mutated version of a previous character in the timeline (hunter). This only occurs if you lost a hunter run to the cycle count reaching 0 (or beyond) and return to the room you died in as gourmand.

Creatures can become temporarily unconscious due to attacks, especially from Spitter Spiders. Also (LORE SPOILERS), Iterators can become basically braindead/unconscious without neurons. Moon is in this state during the time of Artificer and Hunter (the latter of which is on a mission to bring her new neurons), and you can return her to this state later on by eating her neurons. Pebbles also becomes close to this by Saint's time, though he can still talk.

Not literal torture. The game presents nature as it is, mostly animals attacking each other, there's a chance a random animal (like the lizards) will attack the main character, but not carry them back to their shelter, leaving them dead there

You can find baby slugcats in some playthroughs after completing Gourmand's food quest. You can take them, which may count as this, though you basically become their parent. They can also be picked up and potentially killed by creatures if you're unlucky.

(SPOILERS) Hunter's campaign is them using their last cycles of life (they have "super turbo cancer", as the community calls it) to save LttM. Additionally, Pebbles lets you take his last power source in Rivulet so that you can deliver it to LttM so she can regain functionality further.

if you're caught by a lizard creature, they will drag you away to their den. it's possible to get away from them during this if they get distracted or attacked and drop you, but if not, you can't do anything about it. you can skip this sequence the moment you get caught and just reload your game, but if you want a chance to survive, you have to watch yourself get dragged away and wait for an opportunity to escape.

eating enough food before going to hibernate at the end of each cycle is a core part of the game. you have to constantly be finding and eating food, and you're punished if you don't by starvation, not being able to access certain checkpoints, and not being able to pass certain gated areas.

There are tight tunnels that the player crawls through. If the player enters the tunnel the wrong way, they will crawl slower and if they want to turn around it takes a moment of wiggling. The player also ends each day by going into a tiny shelter.
