They first refer to themselves as a girl, later it’s revealed that Eli was born as a boy a long time ago (went by the name Elias) and had forcibly their “lower parts” removed before being turned. Eli uses and is (mainly if not completely) always referred to by female pronouns, but it’s never fully stated what Eli themselves identifies as.
Not during the plot of the book, however it’s worth to mention that Eli was forcibly castrated and tells about that event.
