Audio and visual, but I am terribly etemophobic and I was okay. Toward the end of the film when the gang is at the inn, the 4 men go outside to stop the train. After the flare is fired, Ned tells everyone to run. Look away when the scene cuts to them all heading inside. Joe is the last one in and once he shuts the door behind him, he turns around, bends over and v*mits. Happens super fast so mute when they start heading back inside, and count to about 30 (I don’t have the exact timestamp) and you’ll be all good :)
