If you're coming to this film and story, the rape is an important aspect of the action of the play. But important to note here the rape IS NOT SHOWN ON SCREEN, but here are some possibly upsetting moments surrounding it.
At 55 minutes in the rapists begin to take Lavinia away and there's an extended scene where they drag her around and undress her gradually while they speak and she pleads to Tamora. Once they drag her away the scene ends for now.
1 hour, 3 minutes in they cut back to the aftermath of the rape where she is left alone and disfigured, though the rapists joke at her expense for a moment before running off. The next sequence is the aftermath, and it's relevant for the remainder of the film.
1 hour, 34 minutes in when Lavinia writes the names of her rapists, there's like very artistic, abstract flashbacks to the assault but it's her standing as a deer while the men roar and tigers jump around them. That's the most of the actual assault that's shown on screen.

Lavinia's hands are cut off during her assault (the actual cutting is not shown on screen, but her without hands is). Titus cuts off his hand to save his sons and it is shown on screen in a weirdly comical sequence. The actual cut isn't show, but the amputated hand afterward is show numerous times.

Not technically but Tamora's sons get weirdly physical with one another in their initial scene — humping eachother and burying their faces in eachother's crotch. It verges vaguely incestual but it doesn't do that throughout the film.

I vote no, only because Taymor's intention with the ending is to leave the audience with a feeling that Lucius will be breaking the cycles of abuse that were on display throughout the film. It's an intended hopeful ending, but of course it is one of Shakespeare's tragedies so the majority of the characters die and end unhappily, if that's what you consider a sad ending.
