NO BUT there are several scenes in a very unethical psychiatrist's office and also there are several references regarding sending an alcoholic character off for "a cure" which in those days involved going to a mental institution rather than today's more conventional rehab.

NO BUT, there is a circus act involved with electricity, and an electric chair. It's faked so nothing fatal, but there's one line referring to some muscle pain while performing that act.

YES. The circus's geek performer has a violent freak out near the beginning of the film. At the end, the character who criticized the original geek has become a geek himself and has a violent freak out that mirror's the one at the beginning of the movie. Both are, at a minimum, end stage alcoholics, but both quite clearly have severe mental health problems to the point they are no longer functioning or able to care for themselves.
