Spoiler*
After the orca is slit open and the baby falls out, one of the men looks like he's about to throw up, but nothing happens, which is a relief, especially since I have emetophobia myself.
Later, near the end, when a man is about to throw a dynamite stick, a woman tries to stop him. They struggle, and the dynamite falls to the ground. She quickly grabs it and throws it away. Then she rushes to the edge of the boat, coughing—it's unclear if she's actually throwing up, but it's unsettling, especially for someone like me with emetophobia.

Not a human, but a female orca who’s been harpooned and brought aboard a ship ends up miscarrying- it’s pretty graphic, too
