Yes, someones teeth a very clearly damaged at the beginning of the film (first trap) when their tongue is stuck in a vice. There is a close up of their (front) teeth which are practically nubs at this point.
Yes, but it is a very mixed bag. The movie is about the "blue wall of silence", and is critical of police in very relevant ways. However, it also implies that the problem isn't systemic, there's just a lot of "bad cops", and everything can be solved by getting rid of the "bad cops".

If you’re sensitive to gun violence, I wouldn’t recommend this movie. There are many parts in which people pull out their guns, and explicit gun violence in important scenes near the end.
