The film takes place in a mental institution, which has flashback scenes to the patients being treated in degrading and traumatic ways by doctors, and the party teens generally use degrading and ableist language about/to the cannibals for contextually obvious reasons. The film also is based entirely around the stereotype of mentally ill people as violent and dangerous. The character design for the inbred cannibals is also designed to elicit horror and disgust based on things like syndactyly, albinism, and other real facial and bodily differences which is ableist on its own.

It's hard to qualify it as sad, because it's a slasher film with minimal characterization, but most of the party teens do get killed. It also has a lazy but mean twist where two of the characters (the lesbian couple) escape from the asylum on a snowmobile but are decapitated by a hidden razor wire trap they drive into.
