in episode 9 of the despair arc, an anonymous man is brainwashed into removing his own head with a hacksaw. the entire time, he's screaming in pain and going into full detail about how much it hurts. in the same episode, a major character is forcibly brainwashed by having her eyes held open and having metal picks inserted into her brain while she's fully conscious, watching a video, and screaming in pain. luckily, both of these scenes are easy to see coming and can be skipped. [ SPOILERS ] in episode 10 of the despair arc, a major character is forced to escape a torture chamber shaped like a maze as their foot and leg are impaled by obstacles, their eye is hit with a steel ball, and they're finally impaled by a cluster of spikes. they survive all this, and go through a slow, upsetting death scene in which they say they're not ready to die yet.

in episode 7 of the despair arc, there's an elongated scene that's far more brutally violent than anything ever shown in any of the games in the franchise. the "tragedy of hope's peak academy" (or the first ever killing game) that's mentioned in the first game is shown in total detail, and it's much less organized and much more condensed than any other killing game. from about the 12-minute mark all the way up to the 20-minute mark, there's shooting, stabbing, impaling, strangling, bleeding, chainsawing, coughing up blood, screaming, crying, begging for mercy, and two failed suicide attempts all while an upbeat folk song is being sung in the background. again, this scene is much less stylized and exaggerated than most other death scenes in the franchise, so all of the visuals and dialogue are depicted as very realistic and upsetting. i highly recommend skipping this scene if anything listed above bothers you. i had already played through all the games before watching the anime, and this scene made my stomach turn much more than any murder or punishment from any of the games. it's also worth noting that this scene is shown again in later episodes in the form of a video being played on a screen, although these videos luckily don't have any sound, lessening the severity a bit.
