No, the only mention of any assault is at the very start of the movie some form of documentary or news clips confirms a child was not raped during the abuse she suffered
I might be remembering it wrong,but I believe the teen boy is shot in the crotch in the beginning of the movie. You can see the blood stain there afterwards.
A character has a metal blindfold attached to her head for long enough that when it's removed, skin from around her eyes comes with it, but the actual aspect of her eyes are not focused upon and she does not lose the ability to see from this
I highly suggest not watching this if blood and gore aren’t your thing. That is basically the main point of this movie,that and torture. It’s extremely gory and lots of torture from beginning to end. Keep that in mind,this movie isn’t made for those that aren’t into gore/blood.
The demon-like thing that attacks Lucie turns out to be a hallucination of a woman that Lucie saw while she was escaping. Not necessarily a ghost but Lucie is haunted by her.
Technically no but a woman chained in a room has no other option than to relieve herself using the chair with a hole cut in it with a metal bucket; it’s worth noting she is very upset in this scene
SPOILERS!!!! . . . Towards the end of the film they discuss past martyrs and multiple had cancer of some kind. We see pictures and just learn of their existence & nature of their death.

There is a lot of sounds of rattling/scraping chains, sounds of being beaten, sounds of spoon scraping against a dish when feeding, and metal scraping against metal throughout the movie
A big focus on characters entering an 'altered state' after being tortured and the look on their faces resembles dissociation. Also hallucination scenes

A decent portion of the film features a character trapped in a basement. It is a large basement, however, the fact that it is underground and there is no exit could make one feel claustrophobic.
In the sense of body horror, yes. At certain points we see a very severely scarred woman whose bones sometimes appear contorted. Further toward the end (SPOILERS), a woman's skin is fully removed except for her face, showing the muscles underneath.
Around 33min 50sec until about 36min into the movie, Lucie has a flashback to her escape and during this time the lights in several scenes are flashing/flickering.
