And So Allister was lead to believe that Gozaburo was responsible for his trauma. Same for the other two henchman in this arc. Their trauma was caused by Dartz but he convinces them that he’s here to help them and takes them in.
Technically yes while someone nearly drowns. There is also a scene where a character is trapped inside a burning building and thus is breathing in smoke.
Not sure if this counts but possibly worth mentioning. At the end of the Orichalchos arc, the main villain is fused onto a dragon’s face. The villain becomes what is essentially a shirtless, melted, purple skin version of himself, lacking legs. It’s not too scary but it can definitely be unsettling.

Another possible thing worth mentioning is, the villain of S4, whilst also not necessarily mentally ill, was brainwashed into becoming evil and does kill thousands including his wife, father, child.
