Season 5 episode 17 The Ballad of Lady Frances begins with a very graphic scene of a man being tortured. He is shot and repeatedly asked questions, the man torturing him talks about shooting him in the elbows, knees and ankles and says once he runs out of joints he will "improvise". We both see and hear him being shot repeatedly and he cries out in pain each time. The audio is also heard repeatedly throughout the episode, first in a meeting room where Sherlock, Watson and others are talking to the developers of the technology that picked up the torture, again when Marcus is talking to the victim’s wife and again when Marcus, Sherlock and Watson are talking to the guitar owner in his studio.

After Captain Gregson has been shot, Sherlock has a dream of himself at the captain’s funeral. He is standing by the grave and the temporary captain is speaking to him. Sherlock mentions that he knows it is a dream and that Watson, Bell and a scary clown have spoken to him in the exact same dream. The temporary captain says that Sherlock is nuts and turns to walk away. When he turns away a scary clown is standing there with and axe and swings to axe at the captains head. Ms. Hudson is not misgendered. First of all, she is a trans woman played by a trans woman. Second, they use her preferred pronouns. Third, though Sherlock is slightly sharp with his response to Watson’s saying she “has an Adam’s apple”, he explains there are “stranger things” and continues to refer to her as a woman. Sherlock offers her a place to stay, showing himself to be a trans ally, even paying her to clean and organize when she can’t find work (she’s freelance). The topic is not handled poorly. They also discuss how the meltdown happened in the first place, due to human error. In this fictional universe there is a conspiracy theory that the men responsible for the meltdown where American spies living in what was at the time The USSR, who intentionally sabotaged the reactor, causing it to melt down and hurt a lot of people.
