Sherlock's addiction is consistently discussed and depicted throughout the show; it is an important, recurring point of the series. Several other characters deal with addiction as well.

Ms. Hudson is not misgendered. First of all, she is a trans woman played by a trans woman. Second, they use her preferred pronouns. Third, though Sherlock is slightly sharp with his response to Watson’s saying she “has an Adam’s apple”, he explains there are “stranger things” and continues to refer to her as a woman. Sherlock offers her a place to stay, showing himself to be a trans ally, even paying her to clean and organize when she can’t find work (she’s freelance). The topic is not handled poorly. Sincerely, a trans woman.

Not at all first, but as this is a show centering around murder, and is fairly racially inclusive, multiple POC are seen dead. They are never killed off because of their skin color, however, and they are not typically the victims.

There are several homeless witnesses/victims of crimes, prominently in s2 e15 where the police are incredibly dismissive of a homeless man in distress and without his psychiatric medication. (Spoiler) Watson also mentions that her bio-father is homeless
