I don’t know if it counts, but the only time I can think of is when the protagonist gags a little bit, it’s hard to catch if you aren’t paying close attention. I have severe emetophobia and none of the scenes freaked me out at all.

There is a flashing lights warning at the beginning of the movie, but I'm VERY sensitive to flashing lights and I was fine. There were a few times when you see muzzle flare from people shooting guns and one brief scene with some electrical sparks but they're not as harsh as electrical sparks that I usually see on screen.

This is a difficult question to answer. In a way, the whole movie is about breaking the forth wall, with a video game character becoming aware they're in a video game and talking to the creators and players of said game, but there's no talking directly to the viewers outside of the introductory monologue

*spoilers* The entire town of NPCs is breaking. Guy's best friend is Buddy, and the two of them are running across an ocean. The road they are running on breaks into two & Buddy is on the other side of it, the side that is crumbling. Buddy is destroyed from the game. The rest of the characters are safe and okay. Buddy does eventually come back to life, though.
