Hazel's dad dies in S02 E07 through an IV injection. It's his decision, and it's handled compassionately. Even though it causes Hazel emotional pain, he wants to be at peace rather than dying slowly of cancer.
In the last episode his body is shown cryogenically frozen, and his consciousness is on a computer, meaning it's possible he'll be able to return to life.
