The word itself is not explicitly said, but a lot of the dialogue that can be heard while traversing the overworld implies this (there's a LOT of "(insert character) walking around in that outfit is asking for it").

On your return trip to the Steel Mill, you will find Harley Quinn tied to a pole with her mouth tapped shut. You can optionally repeatedly remove and retape her mouth shut to annoy her.

At least twice in mandatory story missions. One is a sedative, while the other is not explicitly a drug but acts very much like a hallucinogen.

The Penguin is designed to have the end of a broken bottle as a sort of monocle. It is in his face around his eye, although it isn't seen being put there. It can be assumed that it's been there for a while. Unless it is pointed out, it'd be easy to think it was just a normal monocle. While not the eye itself, but around the eye, also, not sure if it could be counted as mutilation.

In the fight against Solomon Grundy, Grundy's ribcage and heart are fully visible. At the end of the fight, Batman reaches into his chest and crushes his heart, which is shown on screen and accompanied by a squelching sound.

None on screen. However, When going through the Penguin's museum, you hear him come on over the loudspeaker, where he freezes an officer's finger(s) with a stolen Freeze gun, and smashes them. It results in an audible shatter and scream.

Unless you want to argue that Arkham City is an extension of Arkham Asylum ,but it’s more of a prison in this case since non-mentally criminals are also held there

PG-13 level language so nothing serious even if it might still catch some off guard to hear Kevin Conroy’s Batman say “go to hell” at least once [edit: ^ those words are PG-13 though like it or not]

Basically every time you get a game over, the antagonist of the current section appears and gives a small speech. It's implied they're talking to Batman, however, it can feel very similar to a broken fourth wall.

Unknown. A positive pregnancy test of Harley Quinn's can be found, but later in the series a child is never mentioned or shown, however this does not necessarily mean that one does not exist and live, nor does it mean there definitely was a miscarriage. At this time, it isn't known for sure, however it may be further explored in the upcoming "Suicide Squad: KTJL". The Arkham spin-off game being developed by Rocksteady, it stars Harley Quinn herself, along with Deadshot, Captain Boomerang, and King Shark (thus far). It's possible they will reveal the true answer there.

the game contains batman-media-typical ableism, there's also a lot of misogyny (specifically coming from henchmen's idle chatter about Harley Quinn) as well as at least one transphobic ""joke"" (again in henchmen chatter, it's all pretty unavoidable)
