The Penguin is designed to have the end of a broken bottle as a sort of monocle. It is in his face around his eye, although it isn't seen being put there. It can be assumed that it's been there for a while. Unless it is pointed out, it'd be easy to think it was just a normal monocle. While not the eye itself, but around the eye, also, not sure if it could be counted as mutilation.

At this time, it isn't known for sure, however it may be further explored in the upcoming "Suicide Squad: KTJL". The Arkham spin-off game being developed by Rocksteady, it stars Harley Quinn herself, along with Deadshot, Captain Boomerang, and King Shark (thus far).
