Sort of? You can see the credits at two different points in the story. One is a fakeout ending where it rewinds and forces you to make the correct choice, and the other comes at the end of the story, but you can return to the game world to complete side missions.

None on screen. However, When going through the Penguin's museum, you hear him come on over the loudspeaker, where he freezes an officer's finger(s) with a stolen Freeze gun, and smashes them. It results in an audible shatter and scream.

At least twice in mandatory story missions. One is a sedative, while the other is not explicitly a drug but acts very much like a hallucinogen.

The Penguin is designed to have the end of a broken bottle as a sort of monocle. It is in his face around his eye, although it isn't seen being put there. It can be assumed that it's been there for a while. Unless it is pointed out, it'd be easy to think it was just a normal monocle. While not the eye itself, but around the eye, also, not sure if it could be counted as mutilation.

Unless you want to argue that Arkham City is an extension of Arkham Asylum ,but it’s more of a prison in this case since non-mentally criminals are also held there

Basically every time you get a game over, the antagonist of the current section appears and gives a small speech. It's implied they're talking to Batman, however, it can feel very similar to a broken fourth wall.

Unknown. A positive pregnancy test of Harley Quinn's can be found, but later in the series a child is never mentioned or shown, however this does not necessarily mean that one does not exist and live, nor does it mean there definitely was a miscarriage. At this time, it isn't known for sure, however it may be further explored in the upcoming "Suicide Squad: KTJL". The Arkham spin-off game being developed by Rocksteady, it stars Harley Quinn herself, along with Deadshot, Captain Boomerang, and King Shark (thus far). It's possible they will reveal the true answer there.

one transphobic "joke" about where a henchmen character refrences a rumour that a female character ""used to be a guy"" (the character is meant to be cisgender but it's still misgendering)

the game contains batman-media-typical ableism, there's also a lot of misogyny (specifically coming from henchmen's idle chatter about Harley Quinn) as well as at least one transphobic ""joke"" (again in henchmen chatter, it's all pretty unavoidable)

Ra's Al Ghul impales himself with his own sword whilst falling from the top of Wonder Tower. Batman dives to try and save him, grabbing him. But, attempting to end Batman (who grabbed Ra's from behind), Ra's stabs himself through, failing to kill Batman in the process.
