Mad Rat is told by the Rat God that she is giving him a chance to fulfill his final wish, and that he will die by the end of the day. While it is true that he will die by the end of the day, she is not even the Rat God as she proclaims. She is a parasite that wants him to be eaten by a cat so that her life cycle can continue. She is even willing to cause him to have hallucinations to get to her final goal.

(Spoilers) This one can depend actually depending on choices you made in the beginning of the game. The human can either be trying to save Mad Rat or just want to do cruel experiments on him. In the very beginning of the game you are given a choices such as "this rat should be killed/saved" or "there are more important things that life yes/no" and those choices determine if Mad Rat was being saved or abused.

The cutscene just after 1-5 (the first bossfight in the game) states that Mad Rat faints due to asphyxiation, which Rat God (the one who explains this) states is common in rats. (SPOILERS. It is not entirely clear whether this is accurate, or simply her attempt to explain why he is unable to move in her presence. END SPOILERS.)

Counting the protagonist rat as a person...I mean, he's not called Mad Rat for nothing. Heart chasitises him for attacking other rats, and it becomes quite apparent that his grip on reality is imperfect. However, once they start piecing together the truth of things, it becomes more properly directed.

The game hits you with cartoony-looking organs almost right out of the gate. In addition, splatters of blood are used in the third boss, a scene where several rats are killed by a cat, and the scene of a horrible accident--as well as a potential consequence of a choice. In addition, the first boss has some grody imagery with brains, and using the time rewind mechanic has a...not exactly pleasant-looking fusion of heart and clock show up.
