Someone yells at Leo for not being like someone he projected onto him, and Leo yells at someone (and I think lunges at them before Mikey & Donnie hold him back) over something that wasn't his fault
Technically, 3 turtles are (2 hinted) dead in the opening scene's timeline. And later its described by another character that they had to eat Rats (never shown)
Adding on to the other reply, 2 anthropomorphic turtles die onscreen, and another gets heavily injured, possessed, and separated from the cast, and another gets beaten repetitively, so it feels like a temporary death for both of them
There is no sex-related-stuff in the movie from what I remember, but there's a scene where a 16-year-old has no choice but to go into a clump thingy of flesh, and has to take off the only clothing (his shell protection and headphones) other than his shorts off in order to do it, and he is very disgusted and scared while doing it, plus the fleshy stuff connects to his nerves
Someone who is triggered by rape may be very triggered by this scene, so if your trauma is really bad, I suggest avoiding this movie If you still wanna watch the rest of it, however, the scene lasts from 58:00-59:00, and the scene introducing the flesh thing lasts from 56:30-57:40
This fleshy stuff is all over the movie, and it may remind someone of nudity. Plus it invades, possesses, and kills unwilling victims en masse, which may also very blatantly remind one of rape. This possesion-like-thing happens to Raph, a big brother and comfort figure in the story
And also, one of the new characters (my fave btw :3) is (huge spoilers) a previous character's son from the future, and while it was never confirmed if he's her biological son, it might remind one of sex and pregnancy, which may also be triggering
He was also raised for the majority of his life by the future version of the main character, Leo. Later in the movie after being permanently separated from the future version of Leo and sent to 2020, he starts to project his dead Dad onto 2020, 16-year-old Leo, which, may be extremely triggering if you think about it too much
Although he doesn't get overly gushy to him, just sort of admires him like the look your cat gives you when you're cutting chicken, and sets unrealistic expectations for Leo Later in the movie he yells at Leo, and, considering their already awkward situation, could also be triggering

Yes, Raph is stabbed in the edge of his shell by a large, fleshy rod. No flesh of his is damaged, but turtles have nerves and blood in their shells, so like yeah
Also when Leo's being beaten by a villian, he punches Leo, who is lying on the ground, with both his fists, it sort of looks like he's stabbing Leo with his sharp, giant thumbs, but he's not.
Kinda, many people get unwillingly possessed by weird fleshy stuff that makes them rabid monsters, so if you think about it too much, it could result in cannibalism
Like.. every 5 minutes... But seriously, I wouldn't recommend watching if 'sacrificing playing a big part in the plot' is particularly upsetting to you. The ending is happy, and everyone survives but it sure does wrack your nerves
Mikey and Leo both die onscreen in the bad timeline within the first 5 minutes, Donnie and Raph are implied to already be dead. Apocalyptic timeline stuff.
In response to other comment: there IS a black character who is dead in the "bad future". They do not die first though, and their death is later prevented.

Kinda, New Character loses all his parents and sent to the past where theyre all teenagers (geez that must be so uncomfortable) there doesn't seem to be any signs of him getting his adult parents back :(
There's a fancomic on tumblr called Cass Apocalyptic Series that covers some of his backstory, and revives 5 of his parents in the end, so read that if you want closure I guess
