Historically yes, the ptolemaic dynasty was maintained through incest, and this is alluded to a few times, particularly when Caesar calls Cleopatra the result of generations of inbreeding. Cleopatra and Ptolemy were siblings. They were also married. They hate each other by the start of the film though and would much rather kill each other than engage in intimate relations so thankfully we do not see any incest in action.
