there are a couple of dogs that have become hollows. While i do not remember if it explicitly states how they die the fact that they are hollows is evidence that they have died.

Kaname Tōsen at one point transforms into a giant insectoid creature.

Numerous bugs are seen to cover Byakuya Kuchiki's body in a hallucination during his battle against Sternritter Äs Nodt.

Nnoitra Gilga's resurrección is reminiscent of a praying mantis.

There is one character in the series who is missing an arm. In the manga version there's a lot of amputation, but it seems to have mostly been censored in the anime.

16x03 through 16x12 Uryu is attacked and spends time in hospital recovering. The others come to visit him. Also worth noting, Ichigo's father, Isshin, runs a clinic, and Uryu's father, Ryuken, is a doctor and director of the Karakura hospital, and often appear in a hospital setting

not really. a character is both mentally and physically unwell after a traumatic experience and it’s implied her stay with the medical staff is due to both of these things, not just because of the injuries. there are no scenes of her focusing on psychiatric treatment, but she is shown unconscious from wounds and receiving treatment. in a later appearance, she is in a frail state and allowed to leave to speak to someone, but becomes distressed/seems to be clinging to a delusion to cope and is sedated (through magic, iirc) and taken back.

not necessarily. one of the main girls is well endowed and has a slim figure. her friends often poke fun at her for the amount of food she eats and that it goes straight to her boobs. i also believe there are a few references to japanese beauty standards

[ep 217] The Espada Charlotte Chuhlhourn wears a short skirt and crop-top and acts like an exaggeration of the stereotypical effeminate man. Yumichika initially laughs at him and doesn't take him seriously. The gag comes back in the TYBW arc when Charlotte comes back as a zombie.

Uryu sometimes gets called four-eyes because he wears glasses. It's usually not meant truly maliciously when said by his friends though. Some comments also about his high intellect, he is the more academically orientated out of the Karakura gang

Both Orihime and Rangiku tend to be sexualised, via clothing (Orihime's TYBW arc dress comes to mind, Rangiku often has cleavage showing), suggestive actions and through the way they talk. Happens to Yoruichi sometimes (naked when training Ichigo and her cat transformation in the TYBW arc) and Isane in the beach/summer vacation filler episode. Sternritter Candice Catnipp also has a very conveniently revealing crop top + short shorts combo uniform

[TYBW arc] Ryūken's mother "adopts" Masaki into the Ishida family and later has the intention of her marrying her son to preserve the purity of the Ishida bloodline, it's assumed some Quincies practice incest to "preserve" their blood purity and status as Echt Quincies. Masaki and Ryūken are siblings by adoption, and neither of them want to marry eachother/both fall in love with other people (Isshin/Katagiri).

The newer Thousand Year Blood-War arc is considerably more graphic in terms of blood than the original anime.
In s2e1, Bambietta cleaves a lower-ranking Sternritter in half, and in s2e6 Haschwalt executes Cang Du and BG9 via beheading, both are very graphic. The Friend episode has some pretty brutal moments in it also.
