Naota is a normal Japanese 6th grade boy although a little cynical. When his older brother leaves for America to play baseball, his brother leaves his 17 year old girlfriend Mamimi behind. Mamimi is sending mixed signals and make advances on Naota, and he doesn't know what to do about her. But to make matters worse, Naota's world is totally turned upside down when he is run over by a woman on a Vespa. During their first encounter, she hits him over the head with her guitar, which then causes a horn to grow out of his forehead. She calls herself "Haruko" and her presence changes Naota's life to even further insanity.

naota is often in compromising and sexual situations with older characters, or touched in implied sexual places without his consent (on the second one, mostly episode 3 with his cat ears)
very heavily in episode 4. naota has a portal in his head that robots come out of every episode, and because of this often has growths coming out of his head, such as big fleshy horns and cat ears. these growths twist and contort when the robots come out. a twisted corpse is seen in episode 4, along with many disembodied heads and a scene where kanon's head is pushed open and haruko's head comes out. additionally, haruko is a shapeshifting alien and often contorts and moves unnaturally (though this is often cartoonish and brief)
naota is eaten by a robot, canti, to 'pilot' him. after theyre done the robot poops him out and hes drawn as a really gross wet poop coming out of the robots butt. first time this happens is at the end of episode 2
mostly off screen, mamimi is repeatedly shown to be bullied in childhood and present. one scene is the aftermath of this, with her wringing the water out of her clothes after her bullies assumedly threw her into the river. its also implied her bullies steal and throw away her shoes.
along with what tial said, one of the major characters is homeless or at least spends a lot of time away from home. she is shown asking for and living off food from other people and spending all her time under or around the town's main bridge, including at night time. she is never shown at home or with parents.
more or less. in episode 4 naota needs to bat away the bomb about to blow up his town, risking his life in the process. he sees it as nigh impossible but does it anyway
haruko and canti leave and naota expresses feeling abandoned and later asks haruko why she left like that. mamimis ex boyfriend also abandons her, and its a major part of the plot and her character
multiple decapitations, which are quite graphic. all in episode 4, near the start when haruko offers to coach naota, and near the end when naota finds his dad, kanon's corpse.
mamimi is in a relationship with someone much older than her at a young age, and later when he abandons her she starts 'pretending' his younger brother is him in his place, having a sexual and inappropriate relationship with him. at the end she stops this and its implied she goes to recover and live her own life
the robot canti, who is somewhat depicted as being 'male', is often seen wearing dresses, skirts and feminine clothing. this isnt really made out as an explicit joke though
in episode 4 its revealed haruko replaced naota's dad, kanon, with a robot thats an exaggerated version of him with all his worst traits naota hates in order to further manipulate him and keep kanon from 'interfering'. this robot isnt necessarily abusive, but is a bad parent to naota. the real kanon is brought back at the end of episode 4
amarao's mouth is stuffed with a rag by haruko while he screams for help, haruko hijacks a hospital and steals worker's clothes, the workers shown tied up and gagged in their underwear.

Naota admits he still has feelings for Haruko despite everything but ultimately doesn’t forgive her, and intentionally creates a situation where she is forced to leave the earth and thus exit his life.
Main plot follows a 12 year old boy being in an unhealthy relationship with a woman significantly older than him, who is manipulating him, doesn’t care about him and only using him to achieve her end goals.
one of the major characters, mamimi, treats 3 other characters as 'replacements' for her ex boyfriend, and extremely emotionally depends on them. one of these characters is a cat (another a robot dog). shes not shown to do anything inappropriate at all to them, but she does push that association on her end.
a cat is briefly mentioned to have almost died in a fire at the start of episode 2 but is saved. this cat becomes a prominent character in the series later on
naota has sexual relationships with haruko and mamimi, who are both much older than him. mamimi is implied to have been sexually abused since she was a kid too, and is shown to have developed a lot of of issues around this.
kind of? a main character has pyromania and in a flash back, sets her school on fire. though nothing is explicitly said or shown, its implied students were injured or even killed during the fire.
yes the robots again, and naota is regularly hit in the head and his head looks 'squashed' in these shots, but his head isnt actually harmed outside a bruise and its very cartoonish. additionally, in a scene in episode 4, kanon's head is pushed open and could resemble head squashing
in episode 4 naota's dad, kanon, is revealed to have been replaced with a robot. in the end he finds the real kanon, a corpse, rotting in a closet and 'reanimates' him (he never really dies, but is depicted as basically a corpse). additionally, naota's mother is absent all throughout, and its said they 'lost their mother' in the light novels. naota is told she isnt dead, but this could be a lie
A character finds out that their boyfriend is cheating on them with another girl while overseas In America, which causes them to spiral into a depressive, nihilistic and cynical state.
about 2 or 3 times, nothing is shown and it isn't depicted sexually at all. though there is a segment after a character has bathed (not shown) and is in a bath towel, which is depicted sexually.
In episode 5, two characters vomit large amounts of water after getting knocked into a river on two different occasions. In the first instance, its very sudden-- the scene before is just two characters talking in a field and it hard cuts to the vomiting.
In the first episode, Naoto goes to the hospital to get the bump on his head checked, only to be attacked by Haruko while on an exam table, having disguised herself as a nurse
Haruko is far from mentally sound and is a very violent, manipulative and completely unpredictable person. Another character displays antisocial behaviour and is a serial pyromaniac and arsonist, but isn’t physically violent
the 'overflowing' before robots come out naota's head can be interpreted as anxiety attacks or meltdowns, they're generally implied to be just 'strong emotions' in general though
Frequently changing art styles and multiple fourth wall breaking jokes. A character asks Naoto what “FLCL” means and when he says he doesn’t know he says “You’re the main character, you’re supposed to know these things”
It is worth noting that in episode 5, Naota's father Kamon is shown wearing a nazi uniform complete with a swastika band on his shoulder, reason why being unknown
in episode 4, kanon's 'corpse' is naked. he is still naked as naota 'reanimates' him by pouring water on him in the bath. see 'is there a shower scene'
regularly and very extreme, though often very cartoonishly. i have ptsd around car crashes and it didnt trigger it at all. also, see 'is someone hit by a car'
lots of violence (usually cartoonish but sometimes not) and some bits of extreme gore, some characters get heavy nosebleeds in some scenes as well. see 'is there excessive gore'
in episode 4 a huge bomb is targeted at naota's town. its in the shape of a baseball being pitched and he has to bat it away, which he and haruko end up successfully doing, and the bomb doesnt blow up the town
episode 5 starts with and heavily features multiple gun fights, albeit mostly cartoonish. the robot this episode is also based around guns, showing a gun loading and revolving before it comes out, and using & shooting ginormous guns when it comes out. episode 6 also has guns and shooting.
