Right at the start, its not really a spider but there are spiderlike legs. I have really bad arachnophobia and it got me relly good, so watch out if you have problems even if its only the legs of one.

Almost at the end, when Jessica realizes that shes still in the other world and Tayler comes to resue her they run away, getting chased by the spider from the beginning.
When they escape through the blue door it gets pushed open again and spiderlegs grabbing Jessica.
