The matriarch of Bey Lah, Hindriarch Keh, deliberately misgenders a trans character. The writing of the game acknowledges that this is an awful thing to do, and Keh is portrayed as obstinately traditional and antagonistic. The quest in which this dialogue occurs - and the area of Bey Lah in general - were created by trans narrative designer Caelyn Sandel.
