[ SPOILER WARNING ! ] When Atou pieces together how Shinano is Origin B, he comes to the conclusion that the doberman carrying Origin B was shot, and when Origin B transferred over to Tina (a shiba inu), Tina was also shot. This is only a description, there is no explicit imagery.
[ SPOILER WARNING ! ] There is a scene where Minoru argues with Rai, and he breaks things in the kitchen out of anger, but I don't know if this could be considered domestic violence since Rai was never hurt.
There is a thwarted attempt at sexual assault in Chapter 7. Noa tries to get on top of Atou without his consent, discussing all the babies they could make together. Atou is clearly uncomfortable and even panicking. This is fortunately thwarted by Hajime.
[ SPOILER WARNING ] Jabuchi burns himself alive after getting attacked by Origin B.
[ SPOILER WARNING FOR ENDING OF THE GAME ] Hatsutori and Utsugi burn together in most endings.
At least not intentionally. Kurachi, one of the heroes, is a cop. Being a police officer contributes to his initial ideas of acting tough in the name of justice. Regardless, Kurachi is only portrayed in a good light out of his willingness to expose corruption in the world. This is not representative of most cops.
(SPOILERS FOR CoE/DLC)
Very few characters break the fourth wall, the obvious offender being Seodore Riddle. Most of his dialogue in CoE's main game includes instructions on achieving better endings. Only at the end of DLC does Seodore speak to the audience with a shown face.
In almost all chapters there is blood/gore present. It is good to note that this game has a pixelated art style though which can lessen the severity of this violence.
