Annie was supposedly abandoned as an infant but it’s later revealed they died before they could come back for her. The circumstances of the other girls at the orphanage are not dwelled into ,but statistically it’s not impossible since during the Great Depression it was considered more humane/practical to give up the child for adoption.

There's a brief mention in dialogue that one of the orphan's parents were killed in a fire, but it's not dwelt on, no details are given and nothing is depicted on screen.

Sort of? A kid falls from one of the lowermost steps at the beginning of "It's a Hard Knock Life." She doesn't get hurt, and you can hear the actresses giggling when it happens.

Miss Hannigan (I might be spelling that wrong) struggles with alcohol addiction and overuse and is abusive and violent towards the children while drunk. Also, some kids very clearly have PTSD from being orphaned/abandoned and physically fight the other kids to cope, sometimes being violent towards the younger ones since they're bigger/older than them.

Ms. Hannigan wants to get it on with the laundry man, she says she wants to "tumble with the bundle". Rooster's girlfriend is sexualized, and when they interact and dance it's pretty steamy.

SPOILER, yes in the sense that Annie will never meet the birth parents she anticipated. People may also be suprised at how Ms. Hannigan is portrayed as a victim of Rooster, it feels like Ms. Hannigan's problems weren't properly solved or addressed--on the other hand she may be suffering from mental illness as I explained earlier, so what can you do? The political conclusions of the movie are different than that of the comic strip that inspired it which may upset some people. They try to tie it all together happily but not everyone will feel satisfied. Annie is privileged and gets to live with the millionaire whereas we're not sure what will happen to her friends.

In "Hard Knock Life" two orphans sing about how they aren't visited by Santa because they live in an orphanage. This contradicts the Santa mythos that Santa brings toys to good children, which these two orphans are, and makes clear these girls don't get a visit from Santa because their parents are dead.
