The “boss fight” scene, which features the man from the start of the movie, physically slaps his son and if I’m remembering correctly he throws his wife out of the way. She also has a facial deformity of unknown origin, it could have been the result of the game or it could have been the husband’s doing. Either way, the wife is greatful when the man is gone strongly implying there was some sort of abuse in the household.

In the “boss fight” scene, it is shown his son has his eye and mouth covered— that part is probably a result of the game, but he also acts fearful and cries whenever he is with his dad. He gets slapped by his father, and tries to kill him when given the chance.

There are three instances. The first being the player to either rewind or fast forward which makes Isaac "throw up" cassette tape. He throws it up, then she chooses reverse, and then chooses fast forward again. The second instance is during The Boss Battle, the player inhales water which forces The Boss to throw up water and the third instance is seen when the police officer sees the guy at the end with the needles. He gags but he comes very, very close to vomiting.
This does not include at the beginning of the film where the son is bleeding out of the mouth from where his tongue had been removed, the blood coming from the mouth of the waitress during level 1 or the blood coming from Isaac's mouth after he had thrown up the cassette tape
