Nothing is shown, but there is insect-like buzzing in several scenes

Lots of long, lingering shots of organs.

So no mutilation, but I need to add this bc nobody else said it, there's a scene where you see a little boy's dead body and his genitals are on full display for multiple shots.

A child is smothered in the first scene. The main character also talks as if he is struggling to breath throughout the film

Not really, but a man is in a sexualized relationship with someone and then kisses someone else

[idk] someone have liquid coming out of his mouth after eating a purple bar and chokes, it happens after the first surgery ( i didn't really see well bc it was triggering to me) also the main character does a lot of throat noises that can also be triggering but nothing happens

Not literally, but the main characters condition is frequently compared or referred to as a cancer

Premise is a society with extremely Ghosh pain tolerance so self destruction is common

Not in the usual way, but a throughline of the movie is that "surgery is the new sex," and there are several sequences of characters being aroused as they're cut open surgically. There's also a fair bit of nudity.

No. But surgery on a certain table is compared explicitly to sex and that table is later used to do an autopsy on a child that feels somewhat sexualized
