A large portion of the movie takes place in a mental institution. The characters frequently visit the same institution, and long scenes take place there. The institution is depicted as dark and creepy.
One character dies from it. A different character attempts it one time, and later dies from it another time. These two characters are frequently criticized and condemned for it.
The characters look directly into the camera during dramatic scenes, but they are not talking TO the audience and are still speaking in-universe. Take that as you will.
