The game takes a lot of inspiration from Giger and Beksinski's art, both featuring disturbing body horror and gore content. Blood, guts, (mutilated) bodies, severed limbs and heads from humanoid creatures are all part of the world in Scorn. The audio amplifies the gore during combat and a few specific cutscenes involving the body of the protagonist.

Near the beginning of the game, around Act 1, a scripted event occurs where the player walks out of a room but suddenly stops. Then looks upward and gets jumped by a mysterious creature that latches itself onto the player in a gorey, brutal way. Prior to this event, the player has seen glimpses of the creature so this might add to the tension, wondering when the creature will appear again.

This world seems to view bodies as expendable and interchangeable. If a body is of no use, it gets quickly discarded and killed off. The player uses a small, disfigured creature for their own gains in-game and can either kill the creature or abandon it when it's no longer of any use to the player.
