Two years have passed since the final battle with Sephiroth. Though Midgar, city of mako, city of prosperity, has been reduced to ruins, its people slowly but steadily walk the road to reconstruction. However, a mysterious illness called Geostigma torments them. With no cure in sight, it brings death to the afflicted, one after another, robbing the people of their fledgling hope.

This depends on which version you are watching. In the director's cut, yes a man vomits geostigma in the very beginning. In the regular editon and special edition this scene is removed compeltely. This is also up to interpretation ofc, but it's been understood that Cloud gets motion sick during his fight with Sephiroth and he gags when he lands on top of the building

No, but the geostigma, and the plotline of mako energy from the original game, are likely allegories for nuclear weapons and the long term effects their use had on those around it as well as the generational trauma it caused. thought it would be worth mentioning even if its not explicit.

There is blood in the director's cut. Depends on which one you watch. Also Tseng and Elena's ID cards have blood on them from when they were attacked. In the director's cut Zack is shown bleeding to death on the cliffside in front of Midgar

Kadaj shoots Elena and Tseng in the very beginning injuring them badly. Cloud is also shot in the face but he is wearing safety goggles so he comes out with a minor scratch
