Douglas is the G3’s director of torture, but tortured no one on screen. Though when under the guise of Dr. Joopy he does go into detail about how his family could be being tortured as you’re helping him. The G3 lab is also conducting fatal experiments on humans and Gatlians. It is shown on screen that Garmantuous’ species can drain the life energy of humans to get high, somehow.

Sort of. You cut off Nipleon’s nipples as proof of the bounty. Knifey also goes into graphic detail about how he wants to kill people.

The protagonist's sister Lizzie does cocaine in the beginning of the game, and the plot revolves around aliens using humans and other aliens as drug sources.

Some NPCs you meet in the game are clearly struggling with addiction. Given that the game is about drugs, it's very likely to be mentioned.

One of the G3 agents you kill, Douglas, is implied to be an alcoholic. While he never drinks onscreen, NPCs frequently talk about his behavior when he's drunk.

One of the executions you can preform with Knifey involves stabbing him into a one-eyed alien's eye and ripping its whole eye out. Can be avoided if you don't stab the aliens with one giant eye. Knifey also talks about wanting to tear out Gene's eyes in his introduction.

Both cartoon and actual gore. Notable examples are when you come across Mopkin which has been telefragged in half with its organs exposed. G3 Grunts will also have their eye explode if finished off with Knifey.

There's a child NPC in the Slums that repeatedly follows and pesters the player. You can shoot him, but it isn't necessary unless you want to 100% the game, as doing so gives you an achievement.

While not visible, some NPCs talk about wetting/soiling themselves when you talk to them. One instance can be avoided by not asking an NPC in Old town where he gets his nickname.

Kenny spits in the player's face so they can understand him. An alien has a butt for a face and speaks in fart noises.

The protagonist's mentor Gene had both legs amputated below the knee, and Lizzie calls him a "cr*pple" during one of their arguments. Some NPCs with drug addictions are heavily stigmatized against.
