Some of the Vigors come with unsettling animations for the player character's hand. These include catching fire and burning down to the bone, the skin splitting apart and oozing blood, crystalline growths protruding from the skin, and octopus-like suckers sprouting from the hands.

One very telegraphed one near the end of the game, involving a Boy of Silence.
Essentially, you lose control of the camera (it forces you to look at a screen), and there is one behind you when you turn around. Definitely not skip the game" worthy.

When Booker and Elizabeth enter the Graveyard Shift (bar) in Shantytown a man walks over to a barrel and vomits into it. You can prevent this by either killing him before he gets to the barrel (he walks slowly), although then you'll have to kill everyone else inside the bar as well, as they will attack you. Or you can walk in and immediatly run down to the basement, that way you'll miss the vomiting.

Booker is a former member of the Pinkertons, a police-like force that was notable for crushing worker's strikes during the 19th century, and he expresses regret over being a part of the organization. The police of Columbia are a major enemy Booker encounters throughout the game.

Being held down under water. It's when you get baptised, when you first arrive to the city and then again at the very end of the game.

People commit suicide after the effects of possession wears off. This is only after possession is upgraded, though.

The game takes place in a very, very racist society. Black people, Native American people, Chinese people, and Irish people are the most commonly targeted.

As Columbia is a very, very racist society, there is a lot of racism targeted towards black people, who are sometimes referred to as "negroes" in environmental signage.

The game is perhaps the most graphic entry in the series. The most gruesome scene happens right before the player is able to do combat. Basically, a man's head is thrown into a rotating hook, with gory results. An innocent man's skin is also eaten by crows. Otherwise the player can perform bloody, but less detailed executions involving decapitations and slashing with the skyhook. Guns also produce bloody results including head explosions.
