Maybe, but probably not. The White Lady is said to have gotten her throat ruined via poison, however, this is only ever mentioned through dialogue/item descriptions, and when you actually meet her she's able to speak just fine(if not with a little rasp to it).
Potentially, the Survivor in Venigni's Factory seems to very much be suffering from some sort of anxiety/panic attack when you come across him. However, this is a minor interaction.
There are no living snakes in the game; however, the alchemists are represented using snake-like imagery, most prominently the ouroboros. There's also a weapon in the DLC with a snake-shaped head("Silent Evangelists' Mace")
Chapter IV takes place in a cathedral, and one of the most important artifacts in the game is called the "Arm of God." Generally, there is a lot of imagery/lore surrounding God(or at least, God-like beings) and angels.
There's one big gator in the DLC that's a mandatory boss, and also one alligator statue in the first chapter of the DLC that you can interact with later on. A minor character is said to have been killed by an alligator as well, though this is revealed(SPOILERS) to be false.
There are several streams, sewers, and a whole area that's one big swamp(Chapter VIII) that you can either choose or have to explore. Basically the entire final chapter in the DLC is also surrounded by a deep sea.
Not sure if I would call it a "mental institution," but the entirety of Chapter II in the DLC takes place in an experimental facility where mental procedures could've taken place, if that counts.
There's an optional shark-like enemy in the final chapter of the DLC you can fight, though it's not really confirmed if it's a shark or just a really big fish. There's also a ship that vaguely resembles a shark.
All the carcass enemies can be seen as body horror, and there's a particular scene by the end of the game that shows an up-close shot of a character's mutilated body(though it's just a bunch of...Blue stuff, it could still be disturbing if you're squeamish).
The final two bosses in the game could also *definitely* be described as body horror. Whereas one has multiple limbs growing in places they shouldn't be, the other is implied to be a puppet made out of a corpse.
Two major characters you interact with are revealed to have been kidnapped later in the game. In the DLC, the main plot revolves around you rescuing another kidnapped character. However, there are no explicit "kidnapping scenes."
General puppets are a regular enemy throughout the game, but more bare/blank puppets are mostly seen in Venigni's Factory and the Barren Swamp. There are also taller puppets present in the last section of Chapter I(DLC) that could be described as creepy if you're afraid of mannequins.
There is an enemy called "The Mad Clown Puppet," who you come across three times throughout the base game(once as a boss in chapter VI, once as an elite enemy in chapter VIII, and once as just a statue in chapter VII). More than that, several harlequin/jester puppets are sprinkled in both the base game and DLC.
Markiona and her Arche Puppets are definitely meant to be seen sexually, but there are no explicit sex scenes or descriptions of sexual content in the game.
Lady Antonia, one of the main characters you interact with, is stated/seen to be terminally ill with the petrification disease. Several other (mostly minor) characters also suffer from this disease.
Several enemies throughout the game wield guns as weapons, and the player can choose to use gun-like armaments as well(ex. Cataclysm, Falcon Eyes, and Pale Knight, though the last one is a gunblade so I'm not sure if that counts)
