Also some minor characters try to sexually assault Sarah and are repeatedly called out as being rapists.
Yes and no. Biff is referred to as "Big Biff" and Paulie calls him a "fat b*****d but this doesn't appear to be explicitly insulting and Biff doesn't appear to have much of a reaction to it, either way.

Not only can you possibly encounter a man dressed like a clown in the Corleone hotel, but a similar clown named Pennies is part of a freeride mission called "Pennies from Hell" and the reward you get from this is a clown suit for the main character to wear in certain circumstances.
Hard to say. No one strictly dies by falling, but special animations sometimes play out where the corpse of a person you just killed falls from a great height.
At the very beginning of the game, once Tommy is telling his story he describes how as a can driver he had a gun pointed at him and was held against his will, forced to drive two people somewhere under the implied threat of death or serious injury.
Depends on your perspective of priceless. Explicitly no. But toward the very end of the game a shootout occurs in an art gallery full of paintings and sculptures that likely will be shot, set in fire and/or covered in blood.
Not only is there a chapter where a character must infiltrate an abandoned prison, but the epilogue in the game shows several scenes of characters being arrested and/or in prison.
Not only can you or nps die or severely be injured to fire when molotov cocktails get thrown, but several scenes, especially the chapter "You Lucky B*****d contain explosions involving fire, often resulting in severe injury or death. There is a particularly gruesome scene that is technically avoidable but likely to occur where an npc graphically burns to death screaming at the player in the chapter "Better Get Used to It"
Not in those literally words but a main character, in the euphemistic language of the game says this. Also you have a random chance of encountering a jumper with a suicide note while driving over the Guliani bridge in "Fair Play"
A character gers their head crushed by repeated stomping in it by another characters foot. While you don't see anything it is later described quite gruesomely.
