I have severe emetophobia and found this movie watchable.
Apart from all of the spitting that happens early in the film (and blood streaming from the corner of a dead woman's mouth after she is lifted by a man) Past the one hour mark, there are two instances of vomiting as the result of poisoned drinks.
The first is a cut to the aftermath of two people who have been poisoned. You see them writhe with black around their mouths and spilled on the floor around them. Given that this is a black-and-white film, a lot of this is in heavy shadow. You don't see the act of vomiting happen.
The second time it happens, it is more extended. A man takes a drink. Shortly after, he fights with another man and it is during that fight that he begins to succumb to the poison. Black blood spurts and dribbles out of his mouth. The sounds are grisly, but it isn't as gross as it could've been.
