Are there heart attacks?

Not directly. In chapter 7 Khan's weak heart is mentioned. In chapter 11 it's mentioned that a specific fictional drug is known to cause heart attacks. In chapter 13 an elevated heart rate is mentioned and a heart attack is used as a prominent metaphor but it does not happen literally. In chapter 19 the heart attack from chapter 11 is mentioned again, and the dot on someone's heart monitor (ECG) figuratively freezes for a moment

Throughout the book the effects of a fictional large scale cosmological effect are sometimes likened to a crashing wave. In chapter 13 a man almost allows himself to be taken away by a wave while surfing.

The relationship between Jesper de la Guardie and his girlfriend Anita Lundqvist is toxic (with a toxic age gap: they began dating when Anita was 15 and Jesper was 30!) but I wouldn't exactly say that they have domestic violence. They do have a fight in chapter 10, when Jesper leaves her. They sling insults at each other. Jesper receives a mark on his wrist during the fight (though the hit isn't actually shown on screen).

One of the main characters, Khan, has a mother whose basement he lives in and she frequently tells him to 'get a real job' (rather than studying disappearances. I wouldn't call Aliyah Khan abusive (she does still help Khan in some of the scenes she is in) but the dynamic could hurt.
Additionally, the parents of the Lund girls (Ann-Margaret and Karl Lund) have somewhat ambiguous attitudes towards their daughters. There may have been some abuse but if there was it was likely not direct (and nonetheless we don't get much information on it).

C11
There is an autistic character who is described fairly bluntly, but the story still treats him well. In chapter 19, a task is described as being done 'with the attention span of an autist.' I would say that the language is dated but the story doesn't treat autistic people poorly.

One of the characters, Deerek Trentmoller, is a convicted child molester. He blames his pedophilia on The Linoleum Salesman, a quasi-headmate of his that may have been supernaturally implanted(?)
In chapter 16 we are introduced to another character who can be interpreted as having DID. He has a headmate he can see who is basically the ghost of a quasi-Frederich Engels / Leon Trotsky figure, who has some misogynistic and homophobic opinions. The headmate is initially portrayed as nothing more than a figment of the character's imagination, but later on there's a reveal that he is actually 'real' when the character is in a location with greater spiritual significance.

I'm not sure if this counts. chapter 9 is a speech by a character to the people of this world. they use the plural you so it includes the reader, but it's an in-universe speech
