Very few, and it is mild. When in an abandoned building searching for the Lizard during a main mission, you are met with a small "jump scare" of the Lizard's shed skin after opening up a storage door.
*Slight Spoilers*
At the beginning of the game Norman Osborn tells his son Harry "I will never let you go", before tendrils start grabbing him as he tries to break free. However, this is only a dream, and Norman is shown to be a fairly regular parent throughout the rest, albeit overprotective at times, and Harry seeming to believe his father cares more for Peter than him.
*SPOILER*
Yes, during the side mission "It Was Meant For Me" Peter is set on fire for a short time by The Flame, however is saved, and doesn't really make any comment on being burnt.
Also The Flame enemy type uses fire as attacks, and certain enemies can throw Molotov's.
*SPOILER*
In the mission "Good Men" Peter is stabbed by Kraven and pretty much dies before the Symbiote brings him back.
Also Hunters use bladed weapons all the time in the game.
Not exactly, but it's worth mentioning that an antagonist deliberately seeks to be killed in battle rather than die of illness. When he is eventually killed by a superior opponent, he welcomes it.
None die on screen, however some deceased reptiles are shown to be in a lab, uncertain if implied experimentation would count, or if they were already deceased before becoming subjects to testing.
Also, I believe mounted animal heads may appear in certain sections.

Yes, there's a joke in the mission "The Flames Have Been Lit" Peter uses the symbiote to stop a hunter from drugging an innocent waiter, and another hunter gets drugged instead. Peter then later uses the same drug to put Dima, Kravin's pet tiger, to sleep.
A major villain is killed on-screen. A minor villain from the previous game is also killed on-screen, and several more are mentioned to have been killed. Some near-death experiences among the major characters.
Both the missions "Not on My Watch" and "New Threads" have the Spiders fighting villains near large bodies of water, not to mention that New York is split into two areas, separated by a large body of water.
*SPOILERS
In the mission "This Isn't You" Peter calls Harry his name, which makes him angry, saying that he isn't Harry, he's Venom. Don't know if it really counts, but to be safe, I'll say that it counts.
*SPOILERS* Kraven's main motivation throughout the story is to find a warrior who is strong enough to kill him, so that he doesn't die of cancer. Kraven does put up a fight, however when given the chance to die, he accepts it entirely.
Very rarely, and when it does happen, it is mild. At one point near the end of the story, a character's head is implied to have been bitten off, showing their bloody necklace fall to the ground.
