In the level "Trail of Blood", the body of a young girl can be found in a house raided by a Mechanist party. The child's ghost will appear and offer you her doll.
While unclear and more implied, a machine child implied to have been made during Karras's human experiments appears in the mission "Life of the Party". While unkillable here, the final mission, "Sabotage at Soulforge", ends with the complete annihilation of Karras's cult, including all the victims turned into mechanized beings, including that child.

The main plot of the game depicts Karras, a local cult leader, buying human beings to use in scientific experiments. He experiments on them, turning them into human-machine hybrids who sometimes audibly, albeit quietly, beg for death to release them from the pain. These experimented-on humans additionally carry gas canisters that, when released, cause humans to be chemically destroyed in painful fashion. The reaction is never shown, but is overheard in the mission "Eavesdropping".

This game is a violent game, and as a result, your player character, Garret, has the option to kill people, and in return, they can kill you. In the final level, "Sabotage at Soulforge", wood nymph Viktoria attacks the enemy base in order to spread enough plant life to sustain a chemical reaction caused by Karras's machines. Viktoria is unable to create enough plant life to sustain the reaction, and rather than fleeing, intentionally allows herself to be killed, her death causing a massive spread of plant life. This allows Garret to ultimately defeat Karras.
