Initially, Konyac and Toba seem to be a regular married couple, complete with sparse suggestions of some kind of sexual activity. This is very clear in the second half of the game, in which that world's equivalent of the two clearly do so.

However, in the late-game return to the manor of the first half of the game, it's seemingly stated that all along those versions of Toba and Konyac were representing the real Toba and Konyac, and were being played by two siblings, retroactively throwing into question such scenes from the first half of the game.

Implied regularly, via the critical gore system (in which enemies and units lose limbs to critical hits) - but these are never shown visually beyond a dark red hit indicator.

In a secret (but still necessary) room much later in the plot, interacting with a sun crest will show a vision of a woman holding entrails.

Beyond a bloody graphic when scoring a critical gore, blood and gore aren't *shown* too often. However, the trappings of the game still get rather violent (as critical gores are just that, attacks breaking some part of their target, etc.)
