Sexual assault (or at least the threat of it) is implied often, and made directly clear a few times.
Some cutscenes involve Nachiroux being grabbed inappropriately, and two of these involve the perpetrators also attempting (and failing) to get her alone.
Eureka has a dream early on in which someone attempts to make her strip-

-which actually happens, though not quite how the dream made it seem, with the character responsible not intending anything sexual.
Also in the latter half of the game, Eureka mentions that she's slept with older men and teachers, despite still being 16.
During a later segment where Nachiroux's store is surrounded by guards, Pumpleton attempts to do so (but Nachiroux escapes.) This is later shown again in a flashback.
Around the same time, when apprehending a serial killer, a character mentions that the killer assaulted some of his victims.

Some enemies are horses. Death is suggested more than shown for all enemies (they tend to just fade away when defeated), but unlocking certain abilities will cause all enemies (the horses included) to drop meat, furthering the implication.

Implied regularly, via the critical gore system (in which enemies and units lose limbs to critical hits) - but these are never shown visually beyond a dark red hit indicator.
In a secret (but still necessary) room much later in the plot, interacting with a sun crest will show a vision of a woman holding entrails.

[Very Late-game Spoilers]
Initially, Konyac and Toba seem to be a regular married couple, complete with sparse suggestions of some kind of sexual activity. This is very clear in the second half of the game, in which that world's equivalent of the two clearly do so.
However, in the late-game return to the manor of the first half of the game, it's seemingly stated that all along those versions of Toba and Konyac were representing the real Toba and Konyac, and were being played by two siblings, retroactively throwing into question such scenes from the first half of the game.

Like most NIS games, there are a fair amount of sexual jokes or items, from some quests offering panty armor items as a reward to characters making sexual comments towards others. Party member designs can also be fairly sexualized, and some cutscenes involve sprites of characters in states of undress (though nothing of detail is shown.)

Whenever a boss enemy is defeated, the screen will turn white before fading back.
Additionally, right before the final boss the effect is repeated but in rainbow colors. Immediately afterwards, the white effect comes back but *much* more rapid and prolonged, bordering on a strobing effect.

[Spoilers]
Nachiroux has an incurable magical disease. It's established that the only help available for it is to slow it down with medication, rather than treat it outright.

There is a magical illness which could be read as similar to cancer, but cancer is not mentioned.
[Spoilers]
Nachiroux has a magic ash bound to her kidney, significantly impacting her health. It is stated to be incurable, irremovable, and the only treatment available is pills to slow down its impact.
