Rusty talks to his therapist about his father's infidelity when he was a child and vowed never to do that to his wife/family. However, he had a sexual/emotional affair with his colleague Carolyn Polhemus who was murdered at the start of the episode. The episode shows evidence of their affair. Autopsy/pathology reports show that Carolyn was pregnant at the time she was murdered.
