In the regular version, no. There’s some blood stains on one page, but not much besides that. The special blacklight edition contains some gorier pages, most notably the stomach-faced duck ejecting its organs.

It’s a book, but it manages to startle quite a bit at one point. The two pages after Fiddleford emerges from the portal are scribbled over in black with erratic red writing and lots of drawings of eyes.

[SPOILERS] This is the canonical source for Dipper’s birth name albeit it’s he himself that reveals it to Ford. And while he tries to assure Dipper that there’s nothing wrong with his birth name he otherwise respects his wishes and only refers to him by his chosen name

One category of ghosts enjoy pain. They have nails sticking out of their heads, and one is depicted hammering nails into the other’s tongue. Also, when Bill possesses Dipper, he purposefully hurts himself quite a bit because he finds pain hilarious, and it’s implied by a coded message that he did the same when he took control of Ford’s body.

A note from Bill mentions that he was planning to make Dipper jump off the water tower and die while possessing his body. He doesn’t get to do so.
Also, on one page, Ford says he felt suicidal (hyperbolically) in reference to a particularly frustrating dimension.

This book actually confirms that Stan and Ford were raised Jewish since their bar mitzvah is mentioned
