I don't remember if there is anything in the game, but the spin-off prequel manga pokes fun at a character for acting/being gay.

Some enemies have spotted patterns that could inspire feelings of trypophobia. The malboro has many "stalks" on its body that end in either open holes or eyes.

Summoning requires the summoners to sacrifice their lives. When used by the player, your characters will die in battle but come back after. It is however shown in story and through dialogue/flavor text that summoning units are essentially suicide squads. Many characters sacrifice their lives for a summoning, including a major character.

Not ghosts, but there are ghost/spirit-like enemies that appear toward the end of the game and, in combination with the atmosphere and environment, can be very frightening.

[SPOILERS]
Nearly all of the player characters die in the end. Because of this, the world is removed from the time loop/cycle it has been in.

There's one moment towards the very end that could be considered so by some. A screen to with a story-significant choice appears, taking up the entire screen as if it were asking the player directly rather than the characters. This moment can also be perceived as unsettling/scary, especially considering the atmosphere of the ending area.

Depends on your definition. The main player characters were raised from very young ages (mostly 7-8) to become soldiers, they are all 16-17 as of the present. Main characters seem to have an unhealthy attachment to the woman who trained them, whom they refer to as Mother, and seem to be poorly socialized for kids their age. Most if not all of the cadets are teenagers/child soldiers and are forced to fight in actual wars, often resulting in their deaths.

A chocobo (large bird-like creature, equivalent to horses) dies in the prologue, and chocobos that you raise can die in combat.
