You take an eye off a puppet and attach it to a foxy mask. The mimic also removes the eyes of some suits but these are inanimate objects. No real eyes are harmed.

The mimic 2 mimics David, Edwin’s son, when David dies suddenly the mimic continues to mimic him and this enrages Edwin, Edwin beats the mimic up to a non functional degree. The mimic then learns violence/is infused with agony and hatred. The mimic then is repaired and kills Edwin while mimicking him. We hear about this through a set of tapes and films. It depends on if you see the mimic as sentient or “actually David” if you would count this as Domestic Violence but regardless it might be uncomfortable for survivors to hear about in game.

Depending on which ending you get.
SPOILERS
One ending has our player character die in a car crash.
One ending the mimic reverts to being David and shuts down.
One ending we give the plans for the mimic to William Afton, which he presumably uses to murder kids with. You are killed in a van while driving.

Head injury happens to a mechanical sentient creature, specifically a door shuts on it, later other head injuries happen from a variety of things. No blood or pain is shown and the creature continues on just fine.
Dollie has a voice line that says “you need a lobotomy!”, her job is to fix the animatronics and robots around the costume manor and this often include things that would hurt humans but don’t hurt bots, though she is actively trying to kill us in this case and it’s not 100% clear if she’s talking to us or just saying a catchphrase

A sentient robot loses teeth, no pain felt. A mini game includes cardboard cutouts of clowns with teeth, your goal is to get the teeth with a ball, knocking all of them down to win.

In the elevator scene after dollie, Fiona says “theres too much weight in the elevator, not to shame, just trying to help you fix the lift” she then goes on to give diet and exercise advice which is an unfortunate lived reality for a lot of fat people, she isn’t making a joke per say but it’s this common though patter of thin people acting like they are our doctors or that simple exercise and dieting will “fix” our fatness. This is wrong for many reasons but especially when we are often fat due to disabilities, poverty, genetics, lack of access to food and otherwise uncontrollable factors. It also reinforces the idea that being fat is inherently unhealthy or wrong morally or that we just “haven’t tried hard enough” which leads to unhealthy relationships with food, disordered eating and even suicides. This is a game set in the late 70s so I’m not surprised at all but a word of warning before watching if you are sensitive to that.

Dollie falls into a smithing vat of molten metal, she is seen badly burned but she is a robot incapable of feeling pain in this situation.
Fiona, Edwin’s wife, died, It isn’t mentioned or shown specifically how she died but hinted she died at the fall test fire. A statue of her is seen burned to represent this

Very minor in this game compared to other games in the fnaf franchise, I’d say more of flickering lights than flashing.
Flickering and turning on of lights during the tunnel sequence
Black flashing/glitching of the screen during theatre area when Edwin reads off what tech they have to replace writers.
When you die, the screen flickers with film reel effect.
In the film room where you collect films, the lighting flickers slightly as you watch the films.
When sleepy moon/moon drop/nighlight chases you in the tunnels sequence, he lights up brightly, this isn’t the issue. The issue is if you are in a dark area and he jumpscares/kill you you suddenly have your screen lit up by his light.

The mimic 1 uses she/her when it mimics Fiona.
The mimic 2 uses mainly it/its but some people in the fandom refer to it as “he”. This could be counted as also the correct pronouns as M2 mimics both Edwin and David (both male) but it depends how you interpret it. Canonically the mimic uses He only when mimicking/using a male costume/person and She only when mimicking/using a female costume/person

I believe there’s some ableist terms like “crazy” and “stupid” used but I could be wrong on this.
One of the logs inadvertently calls the mimic a “psycho”.
I wanted to also note that there is little to no accessibility settings other than colourblind mode and captions/subtitles, which both have flaws.
The captions are often wrong or don’t denote which direction the mimic, the main enemy, is coming from, something that hearing player or players with the ability to use directional sound cues would have playing the game and makes the game significantly harder for deaf players and players not able to use audio. Minecraft is a great example of translating directional sound cues into subtitles.
The “colorblind mode” applies a filter over the whole game and doesn’t actually help with the puzzles that require you to note the green cubes and use the numbers on them, for example other games use patterns to denote the sequence order instead of colours. This filter pretty much only helps with eyestrain and contrast issues with some disabilities and sometimes helps with reading for dyslexic players but typically doesn’t actually help colourblind players much, if at all.
The audio in the sliding puzzle room with the machines that stomp/bang hammers is really overwhelming for neurotypical players, let alone for neurodivergent players and those with sensory issues. You have to complete the puzzle 3 times to collect everything and get to the next section. The audio of the jumpscares/deaths themself are often high pitched, metallic and grating which may affect some with sensory issues as well.
Sometimes the Mimic will say voice lines like “I found you” which do not indicate it actually spotting you at all.
The game requires you to replay it 3 times to get all endings which can be frustrating.
SPOILERS
There are also parts of the game where someone is grief stricken and paranoid (for good reason in this case) and another person tries to push the distressed person into rebuilding his son as a robot to “cope” with his grief, this could be seen as manipulative behaviour, especially as he is in an altered mental state.

It is mentioned that Edwin and Fiona met through their jobs, Edwin being the owner of the company and the relationship having a power dynamic because of this. It is set in the late 70s however, where this power dynamic might’ve not been a big deal.

The mimic, a robot, has its arms and legs ripped of by Edwin, we do not see this but find out through story clues and recordings.
The mimic then rips off the arms and legs of anyone who enters the facility, stuffing them into suits. Though we don’t see this specifically and just see arms and legs of a person sticking out of a costume, looking normal.
There are posters that warn us of putting our hands near heavy machinery stating “limbs replaced at employees expense”
Spoilers
Later the mimic explodes Edwin and he mentions not being able to feel his legs, it is unknown the state of his injuries or if he is alive or dead by the time we enter the story.

The worst parts for me as a trypophobic person was
-when the mimic wears the unfinished mycellium men suits, it has three large black holes on its face.
-when you die to a jumpscare and the burning film reel effect makes a spotty texture.
But generally I don’t think most trypophobes would be upset by this, I am just super sensitive to any patterns of holes/bumps.

There is a parallel to plurality with the mimic being able to mimic and absorb peoples personalities (Introjection) and it becoming murderous because of abuse it faced at the hands of its creator. It isn’t obvious or trying to say anything intentionally bad about DID/OSDD/plurality but the parallel is there for those who want to see it.

There is a first aid scene with dollie the nurse animatronic, she describes how she will “clean” your springlock suit by doing things that will kill a human like you, you then escape.

Our player character can die at various points and means in the game.
Multiple recovery people before us have died to the mimic, we see their bodies stuffed into suits with just a gloved arm hanging out.
SPOILERS
Edwin’s father dies and gives him the company

You can if you walk off of the catwalks instead of fixing them
If you fail at either of the elevator repair games, you either get crushed by the elevator or the elevator falls with you in it, killing you

Mostly small references or allegory to our-world franchises or in-world establishments.
There is some “lampshading” used to reference the lack of windows in the building.

In the opening cutscene, you can also control the steering of the van but you cannot crash.
SPOILERS
in one of the endings you drive away from the manor and the mimic climbs onto your van, you try to shake them off by swerving but the mimic breaks through your windshield and jerks the wheel, making you crash.

Debatable
SPOLIERS
some say the mimic is haunted by agony, kind of like soul juice in this franchise, some say the mimic is non haunted but that it’s AI learned violence.
